CONFIDENTIALITY STATEMENT
LIFE implements practices consistent with HIPAA Guidelines as it relates to Protected Health Information (PHI). This includes maintaining the confidentiality of all clients and their health information in verbal, email and written communications. If identifying information is required to be taken outside of the agency, it must be double locked. In addition, all PHI must be shredded in an agency approved shredder. Employees will undergo an annual training on HIPPA. Our agency also has a compliant officer and HIPAA binder.
